GAVO (Gwent Association of Voluntary Organisations) was established in 1927 and is now the longest-standing and largest County Voluntary Council in Wales. We are committed to strengthening the effectiveness of the Voluntary and Community Sector across Blaenau Gwent, Caerphilly, Monmouthshire and Newport.

In partnership with ABUHB, the Third Sector Link Officer will work with partner colleagues in primary care services via GP Surgeries to support providing patients with the right help at the right time in the right place. This role will be an important part of the Multi-Disciplinary Team (MDT’s) model and it is essential that the post holder has experience of working in the third sector and the ability to collaborate and work in partnership across a wide spectrum of services. The post holder will build good working relationships with surgery staff, allied professionals, community groups and third sector organisations providing the health, social care and wellbeing support in the area.

Newport is a multi-cultural city with its own unique atmosphere and population. Demand for healthcare is growing and will continue to grow. Newport’s Neighbourhood Care Networks’ ambition is to create a new, more integrated system of primary and community care in partnership with our local government and independent/third sector colleagues across Newport.

Bydd y rôl hon yn rhan bwysig o fodel y Tîm Amlddisgyblaeth, ac mae’n hanfodol bod gan ddeiliad y swydd brofiad o weithio yn y trydydd sector ac yn gallu cydweithio a gweithio mewn partneriaeth ar draws sbectrwm eang. Bydd deiliad y swydd yn meithrin perthynas waith dda gyda staff meddygfeydd, gweithwyr proffesiynol perthynol, grwpiau cymunedol a sefydliadau trydydd sector sy’n darparu cymorth iechyd, gofal cymdeithasol a lles yn yr ardal.
