Social prescribing, also sometimes known as community referral, is a means of linking individuals to a range of local, non-statutory services.

FLVC’s Social Prescribers work as part of multi-agency teams and play a key role in contributing to colleagues’  and the public’s expertise about the Third Sector, raising awareness and supporting and signposting to other services.  We have staff based within Flintshire Single Point of Access (for Adults) and the Early Help Hub (for Children and families).  The Social Prescribing role is a key element of Flintshire’s response to the duty under the Social Services Wellbeing (Wales) Act 2014 to provide an Information, Advice and Assistance service.   

Our Social Prescribing service strives to improve the quality of life for service users, by helping them identify ways to build their self-confidence and self-reliance, providing practical support to help them achieve their aspirations, thereby reducing dependency on health and social care as well as reducing social isolation.

This Senior role is a new post  – supporting and working with our expanding team of Social Prescribers.  The postholder will have Social Prescribing duties and will also work with the Department Manager to:

• Support the wider Social prescribing team with day to day issues
• Be a key contact for any Safeguarding concerns
• Help with the co-ordination of our monitoring and evaluation systems to ensure we are delivering a high quality service
• Develop and promote the role of the Single Point of Access/Early Help Hub to all relevant partner organisations
