Social Prescribing helps strengthen community and personal resilience and reduces health and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing, loneliness and isolation, physical inactivity; needs can be wide-ranging.
The Wellbeing Link Coordinator role is a means for GPs and other professionals to refer their patients with social, emotional or practical needs to a range of local, non-clinical services, primarily provided by the voluntary and community sector.
Our social prescribing wellbeing service will be delivered by a Wellbeing Link Coordinator who will work intrinsically with a primary care network and its GP practices, their patients and the CAVS team. The role underpins assistance to those with long term conditions, low level mental health issues, those who may be lonely and isolated, who have lost confidence or have social needs, which as a consequence affects their wellbeing.
