21 hours per week (18 x months subject to further funding)

We are delighted to be recruiting for the position of Young Carer’s Senior Practitioner in our Kensington and Chelsea Personal Budgets project. The service works with young carers and their families, offering assessment and ensuring they receive multi-agency co-ordinated support that leads to longstanding positive change.
 
We are looking for a committed, self-driven, experienced Young Carers’ Senior Practitioner to assess the needs of young carers and their families through home based visits and the allocation of personal budgets that engage young carers with positive activities to improve their health and wellbeing. You will also identify and engage key stakeholders from across the borough offering consultation, advice and outreach training, to assist them in identifying and supporting young carers locally.
 
You will be highly self-motivated, have excellent interpersonal skills and the ability to communicate creatively and effectively with children and young people ranging from 5 to 18 years of age as well as adults with complex needs. You will have excellent communication and presentation skills and confidence in training and developing the skills of a range of professionals.
