Social prescribing empowers people to take control of their health and wellbeing through referral to ‘Link Workers’ who give time, focus on ‘what matters to me’ and take a holistic approach to an individual’s health and wellbeing, connecting people to community groups and statutory services for practical and emotional support.

If you are a creative, innovative thinker, who would love the challenge of working across exciting new partnerships and working collaboratively with DCHS and Voluntary Sector colleagues across Chesterfield North East Derbyshire and Bolsover, this could be the job for you.
You will be employed by Derbyshire Voluntary Action and be part of the ‘Connect to…’ social prescribing team. You will take referrals from DCHS MSK and Personalised Care Clinics and therefore be working closely with DCHS staff the Clinical and Management leads for the project.
You will adopt an holistic, non-medical, person-centred approach to your caseload, co-designing action plans to improve health and wellbeing based on ‘what matters’ to the person and their individual strengths and needs – and supporting them to achieve their goals.
Ideally you will have worked in a similar role before, but this position would also be suited to a DCHS secondment, someone with good local knowledge, appropriate experience and enthusiasm.
