The post holder will be employed by Conwy Connect and will provide and promote an integrated approach to health checks and screening. The post holder will increase attendance of Health checks and Health screening in North Wales for individuals with a Learning Disability. They will work alongside individuals with lived experience and raise awareness of reasonable adjustments with health and social care professions.
