Social Prescribing
£27,730 (Pro-rata)
We are currently recruiting for two Project Leads to manage the Ways to Wellbeing project team and the Primary Care Network team, based at York CVS.
York CVS has been managing a Social Prescribing services since 2015, supporting people who have been referred via their GP/practice nurse or Primary Care worker. Social Prescribing empowers people to take responsibility for their own health and wellbeing by linking them to non-medical support from within the community. We take a holistic approach and provide person centred support to individuals to improve their health outcomes.
We now offer Social Prescribing to individuals accessing statutory mental health services, hospital discharge and within the Community Safety Hub. These are new and innovative areas of delivery reflect the development and growth of Social Prescribing across the city of York.
The successful candidates will have significant experience of working in the healthcare, social care or voluntary sector with vulnerable adults. They should have excellent communication skills and good organisational skills with a minimum of one year’s management experience in a similar setting.
Please note that this role may require occasional evening and weekend working.
