About the role:

An exciting opportunity to work as part of a multidisciplinary, integrated team. You will empower people to take control of their health and wellbeing and focus on what matters to them, through creating support plans in partnership with service users and carers. Taking a holistic approach, you will connect people to community groups and statutory services for practical and emotional support.

About you:

- Knowledge of relevant legislation and recent developments in social care, and working knowledge of vulnerable individuals within the statutory, independent or voluntary sector.
- Ability to undertake social care assessments, provide outcome focussed support plans and monitor and review outcomes.
- Excellent communication skills, with a strong ability for partnership working.
- Great team working skills and ability to organise and prioritise your own workload.
- Flexible approach to your working pattern, and mobile and able to work from various locations.
