Social Worker / Care Manager

Position: Social Worker / Care Manager - Long Term and Review Team 
Location: Rutland, East Midlands 
Job type: Full Time, Permanent 
Hours: 37 hours per week 
Salary: £24,799 to £34,788 per annum; plus market supplement of £1,000 - £3,000 pro-rata for SW qualified 

The closing date for applications will be the: 05 August 2019

About you: 

You are experienced in an adult social work / adult health environment, have a person centered, preventative approach and you are passionate about making a difference. You will have an understanding of the personalisation of services agenda, and the implementation of self-directed support services and be able to deliver the statutory functions of The Care Act, manage a caseload and be part of a motivated and highly experienced multi-disciplinary team. 

The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS).
