Social Worker / Care Manager

Position: Social Worker / Care Manager - Hospital Discharge and Reablement Team 
Location: Rutland, East Midlands 
Job type: Full Time, Fixed term to March 2020 
Hours: 37 hours per week 
Salary: £24,799 to £34,788 per annum plus market supplement of £1,000 to £3,000 pro-rata for SW qualified

About the role: 

Passionate about providing and developing excellent services, you will be comfortable working in hospitals (typically in Leicester, Peterborough and Kettering) and in service users homes to assess needs. Able to quickly build relationships, ensuring they have all the support they need to regain their independence, return home and remain there safely, will be your starting point. Working in a person centred way will be second nature, as will ensuring support plans are monitored and reviewed to meet changing circumstances. 

About you: 

With previous experience working in Adult Social Care, you will understand the processes and legislation behind their work, but just as importantly you will support service user’s involvement throughout the assessment process, creating plans in partnership with them, their careers and other agencies to facilitate safe, effective and timely hospital discharges and transfers of care. 

To ensure they offer a holistic service, they would like to hear from qualified Social Workers and other professionals with a relevant Level 4 qualification or equivalent such as Nurses, Physiotherapists or Occupational Therapists to work in the team as a Care Manager.
