Prospectus is delighted to be partnering with The Grace Eyre Foundation, a charity who support people with learning disabilities and/or mental health needs to enjoy personal freedom, be respected for their contribution, and be treated as equals within the communities where they live. They are currently recruiting a Shared Lives Coordinator to join their London based Shared Lives scheme. This role is offered on a Fixed Term Contract until 31st May 2020.

As the project's Shared Lives Coordinator you will work across the London Borough of Lambeth assessing and supporting suitably matched carers and service users. With a focus on enabling people with social care needs to achieve positive outcomes, your role will entail a dual approach. Firstly, you will be responsible for providing advice and guidance to newly attracted carers to the scheme as well as conducting detailed assessments to establish their suitability, interests, lifestyle and background that will contribute to a successful placement match. In addition to this, you will also be responsible for providing on-going support to a caseload of placements acting as a first point of contact for allocated carers as well as conducting regular visits focused on ensuring the needs of service users and carers are met.

To be successful in this role, you will previously have managed a caseload and have had interaction with individuals with addition social care needs. With strong communication skills, you will possess the ability to conduct assessments including identifying risk and making recommendations to a board of decision makers as well as possessing a strong capability of completing support plans that are person centred and SMART. Acting as a representative of the charity, you will have previous experience as a multi-agency worker and understand the benefits that this has on service users and carers a like. Finally, an NVQ level 3 in Social Care and knowledge of the Shared Lives Scheme would be ideal, though they are not essential.
