Location: West Midlands
Salary: £13,500
21 hrs per week, home based. Hours to be worked flexibly by agreement, mostly Monday-Friday (possibility to work 4 specific days by negotiation).

Our client's Family Support Workers offer practical support to families with VI children. 

With outstanding communication skills and the ability to manage caseloads, their Family Support Workers deliver the services predominately via telephone support, and home visits when required.  Alongside developing relationships with a wide range of external and internal stakeholders you will source referrals and reach families who are most likely to benefit from the service, ensuring they can access any information they may need.

Their ideal candidate will have extensive experience of supporting families, be highly motivated and able to communicate effectively with families and vision impaired children. In addition, they will be skilled at networking with other services and professionals and have intermediate level IT skills including Excel.

Due to the nature of this position, the ability to travel within the West Midlands and occasionally to their London office - is essential.

PERSON SPECIFICATION

Essential
Educational or Professional Qualifications
Educated to Level 3 or above in a relevant professional qualification (Education, Social Care, Health) or relevant experience.

Experience:
•    Extensive experience of working with families in need of support.
•    Experience of working with local professional partners.

Knowledge:
•    Knowledge and understanding of family systems
•    Knowledge of SEND Code of Practice 
•    Knowledge and the willingness to further develop understanding of vision impairment and the impact it has on families
•    Good knowledge of professional roles and expertise in wider services that impact or interact with families and VI children

Skills:
•    Ability to problem solve including supporting individuals to access appropriate services
•    Excellent organisational skills including managing a case load
•    Ability to work independently and from home.
•    Excellent team player
•    Ability to communicate effectively with families, children and young people
•    Good listening skills
•    Competent IT skills including Outlook, Word and Excel
•    Excellent oral and written communication skills including report-writing

Desirable: 
•    Experience working with VI children young people or their families

Conditions of the post:
•    Travel to meetings and for service delivery


Primary Objective
To support families with children and young people (0-25) with vision impairment to ensure they access services they need to meet their needs

Key Tasks and Main Duties:
•    Assess the most appropriate way to support families with VI Children i.e. through home visit, telephone, email
•    Identify any families who are in need of interventions that support family systems and introduce them to a Family Engagement Worker of their society
•    Agree and monitor progress toward goals through action plans agreed with families ensuring a clear exit strategy 
•    Make referrals to relevant services and follow up to ensure families receive the support needed from the most relevant body to meet their needs
•    Provide information regarding local and national services which will help to support families and VI children
•    Provide support to complete forms, (e.g. benefits/funding), attend meetings (e.g. EHCP Annual Review) and similar practical support in response to identified needs
•    Establish and maintain good working relationships with relevant services that impact on VI families
•    Attend local Eye Clinics where agreed with Family Engagement Workers to promote the society activities to families 
•    Comply with departmental reporting systems and processes including but not exclusive to changes in circumstances of families/children, new referrals, data collection ie contacts, activities, links to local services, family action plans and notes from interactions with families or children and young people.
•    Manage own case load effectively
•    Implement organisational policies and procedures in practice
•    Utilise the organisational information and reporting systems to ensure consistency in information and reporting.
•    Provide regular case studies demonstrating the impact of the work within the timescales set out in the service requirement.
•    Comply with the organisation guidelines in relation to GDPR rules as defined by the organisation. 
•    Keep up to date with current legislation that may affect ways of working with children, young people and their families.
•    Work collaboratively across the services team and the organisation participating in team, departmental and organisational meetings to learn and share information, knowledge and expertise
•    Ensure that equality and diversity is embedded in all activities
•    Ensure all service delivery conforms to Health and Safety regulations and safeguarding standards
•    Ensure compliance with safeguarding policies and legislation and protect the welfare of service users and all other children and young people/ vulnerable adults that have contact with the organisation
