Our client is the UK's leading dementia charity. They provide information and support, improve care, fund research, and create lasting change for people affected by dementia.
Position: Memory Support Worker
Location: Leeds
Contract type: Full time, Permanent
Hours: 35 per week
Salary: £20,345 actual per annum
Closing Date: 27 June 2019
Interview date: Week beginning 8th July
About the Role:
Our client is working in partnership with Leeds and York Partnership NHS Foundation Trust to deliver an innovative service where Memory Support Workers have the rewarding opportunity to provide support and guidance to connect the person with dementia, and their family/carer, to community support; and integrating primary, secondary and community care in a person-centred approach. The team will support the management of dementia in primary care and connect directly to initiatives to reduce admissions and maintain personal well-being & independence for as long as possible.
About You:
They are looking for an individual who possesses minimum Level 2 Health and Social Care or relevant qualification with an excellent understanding of dementia and the needs of those affected by it. Your ability to provide person centred assessment of need, along with a clear understanding of the need for client confidentially is essential to providing a valuable and worthwhile service. To be successful in this role you will have excellent non-judgmental communication skills as well as strong organisational and time management skills in order to manage your caseload of clients effectively.
