About their Transition Service:

Support Workers at the Transition Service ensure that the people they support gain the skills and knowledge to live as independently as possible and support them throughout this transition.

About the role:

In these roles, everything you do will enable people to have choices about their own lives, creating flexible services that meet people’s needs and always looking for the best way of doing things.

Within this service you could be supporting someone to the local shops, supporting with meals, attending medical appointments, supporting to school/college and supporting with activities in the local area. You will also encourage and promote independence and support with life skills such as cooking, cleaning and banking.

You will need to be able to work shifts, including weekends, some nights and Bank Holidays. Shifts are on a 7.5 hour basis and start between 7am up until 10pm.

About you:

You don’t need experience of support work or any special qualifications for this job - you’ll get plenty of training along the way. What matters most is your positive attitude to life and your desire to support people to live their life with as much independence, choice, dignity and control as possible.
