Support Worker 

Take on dementia together 

Our client is the UK's leading dementia charity. They provide information and support, improve care, fund research, and create lasting change for people affected by dementia. 

Position: Dementia Support Worker 
Location: Homeworker South Cheshire (Crewe and surrounding areas) 
Contract type: Part time, Permanent 
Hours: 28 per week 
Salary: £16,276 actual per annum 

Closing date: noon on Monday 19th November. 
Interview date: Monday 26th November 

About the role: 

This is an exciting opportunity for a self-motivated, compassionate individual to join the Cheshire East team as a Dementia Support Worker, working closely with GP practices and the memory service in the Crewe area to deliver high quality one to one information, advice and support for local residents affected by dementia. 

The role is home based but applicants will need to be able to travel to their Warrington office for induction and training 

About you: 

You will be working in partnership with colleagues from a variety of organisations across the health and social care sector, to provide person- centred information and advice to people living with dementia and their families and carers. 

You will be able to build effective relationships with service users, through a variety of communication methods, supporting them to identify their needs and to access local services. As a consequence, enabling them to have choices, maintain their independence and improve their sense of well-being. 

You will be required to manage referrals and maintain a caseload logging accurate, up to date support plans and contact records for service users. 

This role is funded by the South Cheshire Clinical Commissioning Group.
