Support Coordinator (Stroke Charity)

Position: Stroke Association Support Coordinator - Communication Support 
Locations: Brighouse office; covering Kirklees area - North Zone 
Job Types: Fixed term until 31 March 2019 (Our services are contracted; we currently have funding for this contract until 31 March 2019). 
Hours: 30 hours per week 
Salary: circa £19,700 per annum

About the role: 

They’re looking for an innovative, passionate and professional individual to join their team in Kirklees. Working closely with the hospital and community multi-disciplinary teams you will provide emotional and practical support to stroke survivors, their families and carers, both in hospital and at home. 

The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, providing a range of innovative support solutions. A key part of the role will be to recruit, train and manage a team of volunteers, to support service users and the delivery of the service. Referrals are received from the Acute and Community Stroke Teams in the local areas, families and other healthcare professionals. 

About you: 

Applicants are likely to have a background in the caring profession, preferably with knowledge of stroke. You must have all round flexible skills in listening, communication, organisation, IT and resourcefulness. 

The use of a car with business use cover and the ability to drive is essential to be able to fulfil the requirements of the role. 

You may have experience of the following: Communication Support Coordinator, Service Coordinator, Health Care, Stroke Recovery, Third Sector, Charity, Charities, NFP, Not for Profit, Support Coordinator, Advice, Support Coordinator etc.
