Support Coordinator

Position: Support Coordinator 
Location: West Cumbria; homebased, North Zone 
Job Type: Fixed term until 31 March 2019 
Hours: 21 hours per week 
Salary circa £13,800 per annum

They are looking for an enthusiastic and motivated individual to join their team in West Cumbria. Working closely with the hospital and community multi-disciplinary teams you will provide emotional and practical support to stroke survivors, their families and carers. 

Reporting to the Support Manager, the Support Coordinator will provide support and information to stroke survivors and their families both on the ward and at home. By assessing the needs of each stroke survivor and their family, you will aim to reduce the risk of further strokes by supporting people to make lifestyle changes based on their identified risk factors, along with helping them to set personal goals and work towards them. 

The service operates within Cumbria and the surrounding areas, with referrals being received from the Community Team, families and other healthcare professionals. 

Applicants are likely to have a background in the caring profession, preferably with knowledge of stroke. 

You must have all round flexible skills in listening, communication, organisation, IT and resourcefulness. 
The use of a car with business use cover and the ability to drive is essential to be able to fulfil the requirements of the role. 

You may have experience of the following: Support Coordinator, Service Coordinator, Health Care, Stroke Recovery, Third Sector, Charity, Charities, NFP, Not for Profit, Support Coordinator, Advice etc.
