Position: Support Coordinator 
Location: Bracknell Forest; office based in Bracknell 
Job Type: Fixed term contract until 31 March 2019 (Their services are contracted; they currently have funding for this contract until 31 March 2019) 
Hours: 35 hours per week 
Salary: circa £23,000 per annum

About the role: 

They are looking for an innovative, passionate and professional individual with excellent interpersonal and organisational skills to join their support team in East Berkshire. The role will be office based in Bracknell with regular travel predominantly within Bracknell Forest, but also across East Berkshire and further afield. 

Reporting to the Support Manager, the Support Coordinator will support new survivors and their carers from hospital discharge into the community. The coordinator will provide personalised information, advice and support with regular reviews, including a review at 6 months which forms part of the national stroke audit. 

About you: 

You will ideally have a background in a caring profession with experience of supporting people with disabilities. As you will be accountable for volunteers, experience of recruiting and managing volunteers is a benefit. 

You will have excellent IT skills and demonstrate a flexible approach to your role. The use of a car with business cover and the ability to drive to fulfil the travel requirements of the role are essential. 

This is an exciting opportunity to work with stroke survivors and their families to support them following stroke and to raise stroke awareness in the local population. 

You may have experience of the following: Stroke Recovery Service Coordinator, Support Coordinator, Service Coordinator, Health Care, Stroke Recovery, Third Sector, Charity, Advice, Administration etc.
