Position: Support Coordinator x 2 
Location: 1 role office based in Reading and 1 role home based 
Job Type: Permanent (Their services are contract based; they currently have funding until 31 March 2019) 
Hours: Part time, 28 per week 
Salary: circa £18,000 per annum

About the role: 

A fantastic opportunity has arisen for two passionate and professional individuals with excellent interpersonal and organisational skills to join their Stroke Recovery Service team serving Reading and Wokingham. 

One role is office based in Reading; the other will be home-based predominantly serving Wokingham. Both entail regular travel, predominantly in and around Reading and Wokingham but also further afield. 

Support Coordinators balance busy caseloads and work across a range of care, community and home settings. You will provide regular reviews and create client-centred intervention plans to support people to achieve their desired outcomes. The ability to provide clear, personally tailored information and advice is essential. Experience of recruiting and managing a team of volunteers would be highly beneficial. 

Reporting to the Support Manager, the Support Coordinators will join an innovative and vibrant team in supporting stroke survivors and their families from hyper acute stroke admission through to adapting to life after stroke. 

About you: 

You will thrive in a fast-paced environment with a proven track record of effective time management. You will have a background in a caring and/or charity profession supporting people with disabilities. 

You will have excellent IT skills and a strong understanding of how to use data to improve quality. 

A flexible approach to your role, the use of a car and the ability to drive to fulfil the travel requirements of the role are essential. 

You may have experience of the following: Stroke Recovery Service Coordinator, Support Coordinator, Service Coordinator, Health Care, Stroke Recovery, Third Sector, Charity, Advice, Administration etc.
