Administrator - Referral and Outcome Worker
The new Community Connector Service builds on the work of Social Prescribing and Village Agents across the county. This new exciting service seeks to make use of the strengths in the local community by working closely with the voluntary and statutory sector and connecting individuals to services that can support them
We are looking for a Referral and Outcome Worker to be based at our Offices in Stroud.
You will be responsible for providing an effective and confidential administrative support to the Community Connector Service
You will work closely with Service Delivery Manager and Senior Community Connector to develop administrative systems which ensure the Community Connector Service is able to work efficiently and record data effectively
You will be single point of contact for referrals into the service and undertake follow up calls to clients
A key part of the role will be the ability to deal efficiently, professionally and courteously with clients, members of the public and external agencies contacting the Community Connector Service.
The post holder will play an important role in supporting the Community Connector team who will work across Stroud Locality.
