About the role
This is a fantastic opportunity to join the British Red Cross within the Crisis Intervention Community Support Service (CICSS) team providing reactive, focused and flexible support to vulnerable adults at a time of worsening health crisis. The post will assist the service co-ordinator and assistant co-ordinators by undertaking duties required to meet the service users' needs in relation to worsening health and in order to re-enable service users to remain in their own home during their health-related crisis period. The post-holder will respond to referrals from Clinicians and Community Health Professionals in Nottingham and will be required to have the ability to assess service users' needs and complete a care plan for the individual.
The post-holder will need to offer personal care, medication prompting and signposting practical emotional support in the service user's own home.

About the team
The Crisis Intervention Community Support Service is focused and intense within the
Nottingham North East and Nottingham West Clinical Commissioning area of Nottingham. The post will to support the work of the Crisis Intervention Community Support Service and work alongside our long established Hospital to Home service at Queen's Medical Centre Campus at Nottingham Hospitals Trust.
About the person
- NVQ Level 2 or equivalent in Health & Social Care
- Experience of working in a Health & Social Care
- Full and valid driving licence
- Full daily access to a car
- Experience of delivering personal care needs
- Participation in a multi-disciplinary team environment
