Service Support Worker - Home from Hospital
Location: Ceredigion, West Wales
Contract type: Fixed Term until Fixed term, March 2018 (subject to extension)
35 hours per week
Salary: Circa £15,000 per annum pro rata
About the role
We are looking for an enthusiastic and passionate individual to join our dynamic Independent Living Team as a Support Worker. This is a great opportunity for someone who is a people person and has confidence and knowledge of supporting vulnerable people with different needs and requirements at various stages of their life.
You will support the delivery and development of the service in Ceredigion. You will provide and deliver support across a 7 day on-call rota system, tailored to the individual needs of each service user (will only work 5 days in every seven days). You will undertake tasks related to the practical and emotional needs of service users following an assessment of need and will provide advice, encouragement and practical help that will promote service users' recovery, confidence and self-esteem enabling them to maintain/regain their independence.
About the team
The Independent Living & Crisis Response service (IL/CR) supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency; maintaining an extensive network of external relationships across health, social care, and emergency services
About the person
To succeed and thrive in this role you must be organised, ethic and motivated and have following skills and experience:
- Excellent communication skills
- IT literate
- Educated to GCSE level or equivalent by experience
- QCF level 3 health & social care qualification or achievement of this within 18 months of commencing
- Knowledge of locality served by the scheme
- Understanding of issues faced by vulnerable people
- Understanding of needs and expectations of vulnerable people who have health & social care support needs
- Ability to manage own time and workload and respond to and prioritise a range of competing demands
- Able to drive, hold full clean UK driving licence and own/have access to car which you are willing and able to use as part of your duties
