Location: Warmley, Bristol and South Gloucestershire
Contract type: Permanent
Full Time (35 hours per week) or part time (negotiable)
Salary: Circa GBP 14,500 (FTE) per annum
About the role
We are looking for an enthusiastic and passionate individual to join our friendly Independent Living Team as a Support Worker. This is a great opportunity for someone who is people person and has confidence and knowledge of supporting vulnerable people with different needs and requirements at various stages of their life.
You will provide reactive, focused and flexible support to adults either following a hospital stay to enable early discharge when clinical needs have been met or to those living in the community to prevent hospital admission. You will assist the Independent Living Service Coordinator by undertaking duties required to meet the service users' needs in order to re-enable service users to remain in their own home and prevent hospital re-admission. You will be providing practical and emotional support in the service user's own home and signposting to other services where appropriate to enable the individual to live independently in their own home
About the team
Bristol First Call provides practical and emotional support to people over the phone and/or in their own home. The service provides a range of support from one off telephone calls for advice and guidance, visits at home for those who need them and also a supported resettlement service to support with discharges from hospital, providing transport and resettlement at home
About the person
To succeed and thrive in this role you must be organised, ethic and motivated and possess following skills and experience:
- Ability to work as part of a team and on own initiative
- Able to plan own workload
- Educated to GCSE level (or equivalent by experience)
- IT literate
- Knowledge of supporting service users who may be disabled and/or frail, impaired and have dementia and mental health issues
- Working with individuals with a sensory or physical impairment or who may be frail due to age
- Participation in a multi-disciplinary team environment
- Willingness to work flexible hours
- Full driving licence holder and access to vehicle
